What to expect at your first visit
Your first visit is all about learning who you are. We want to know your story, your desires, and your priorities. As a new patient, you will meet with your new dentist one-on-one. Together, you will discuss your expectations and concerns prior to beginning treatment. Your new patient appointment will include:
Reviewing your medical and dental history
Discussion regarding your expectations and top priorities
Radiographs if necessary
Full dental exam
Full periodontal exam
Hard and soft tissue exam
Oral Cancer screening
After your comprehensive exam is completed, we will either schedule your treatment or, if there are multiple options to be considered, a follow-up consultation with your dentist.
Prepare for your first visit
Complete your medical history and new patient packet, a link will be provided for you via phone when scheduling the appointment. If you choose to complete the paperwork in person, please arrive 15 minutes early.
Forward any current records or radiographs (within 1 year) from your previous dentist. They can be emailed to greenfieldsmiles@gmail.com. If forwarded radiographs are current but not diagnostic, they can be taken at your appointment.

Whether we're a preferred provider for your policy or not, we'll always work on your behalf to help you get the most out of your benefits while still providing the highest quality of care. As a courtesy, we will submit claims to your dental insurance company on your behalf.

For those patients who are uninsured or underinsured, we offer options to make quality dental care accessible and affordable. Our options include:
One low monthly fee allows 2-3 cleanings per year, 2 exams, routine x-rays and 1 emergency exam at no charge (up to $349/year savings), as well as a 20% discount on additional services at our office. The best option for patients with no dental insurance.

Our office policies are in place to ensure that all patients receive the highest quality of care and fair, timely, and appropriate treatment. Thank you for your cooperation and understanding.

You will receive reminder messages at set intervals before your scheduled appointment with us. Please respond to confirm but do NOT opt out of messages. If you opt out of reminders, we are no longer able to text you for any reason. This is our primary mode of communication for dental emergencies after hours and weather cancellations, etc. If you do not receive reminders, you do not have a scheduled appointment.

We understand that last minute cancellations or sicknesses are sometimes inevitable. You will not be penalized for a first time occurrence. After a first missed appointment or same day cancellation, fees will be assigned at manager’s discretion based on circumstances and the amount of disruption to the practice schedule.

Payments are due at time of service. If you are utilizing dental insurance, we can estimate your out of pocket responsibility prior to a service being provided. Your estimated out of pocket responsibility is due at time of service. This estimate is based on your specific insurance and is not a guarantee of payment. If your actual responsibility is higher than estimated, you will be billed accordingly.

Although we submit claims as a courtesy, your insurance benefits are ultimately a contract between you and your insurance company. You are responsible for knowing your benefits and coordinating payment if we do not receive it. We will submit claims initially and resubmit with corrections if needed within 6 months. After 6 months of no payment by insurance, the balance will become your responsibility. If you remit payment and your insurance company does as well at a later date, you will be refunded.

Interest free payment plans are offered for services to help ease the financial burden of your treatment. The monthly installment offered will be based on the full cost of treatment. Payment plans must be set up by the day of service and require a credit card to be placed on file for payments. Plans can be revoked at any time if payment terms are not being satisfied.

If a refund is determined to be owed, it will automatically be placed as a credit on your account to be utilized for future services. Upon request, your refund can be mailed to you via check within 10 business days.

Should you require your records be sent to another office, please allow 2 business days. We will automatically provide any current radiographs (BWX within 1 year or FMX within 5 years). If any further information is required, we ask that you have the office reach out to us directly. We will request that you sign a records release form.
